I/we give Jogjakarta Community School permission to use my child's photograph:

• On the JCS website, JCS Facebook, and in social media
• In any published or printed school marketing materials
• In any communications to the JCS community and external bodies.

I/we understand that:

• Photographs will only be used for school-related activities.
• I/we will not be compensated for the use of my child's image.

I/we free JCS Board of Management (Yayasan), Administration, teachers, voluntary helpers and employees, from financial liability in connection with my child during attendance at the school and during any related educational activities.

I/we give permission for my child to go on any field trip which the school has organised.

JCS will send parents/guardians a notice of these proposed trips at least one week in advance. Parents/guardian must inform the child's teacher if he/she does NOT want his/her child to attend.

NOTE: For any excursions requiring overnight camps or stays, a special notice and permission slip will be sent for signing and return.

I/we give permission for emergency measures to be initiated in case of an accident or sudden illness. In the case of an emergency, I/we authorise JCS personnel to have my child transported to Jogjakarta International Hospital or the closest reasonable facility and accept the charges that may be incurred.

I/we agree that all information is disclosed and correct. I/we understand that failure to disclose relevant information related to the child may result in the exclusion from JCS.

I/we agree to comply with the JCS Code of Conduct which is outlined in the JCS Parent and Student Handbook. I/we understand that violations of this Code of Conduct may result in the exclusion from JCS in accordance with the stipulations outlined in the JCS Parent and Student Handbook.
